Liabilities : Student hereby acknowledges that the course of instruction is physically rigorous and that he/she is capable of participating in the course of instruction.

Assumption of Risks: Guardian and/or Student understands that martial arts is a contact art/sport and are/is aware of inherent risks of injury, death, and property damage involved in martial arts instruction, training, and competition, including, without limitations, risks due to contact with other students and instructors. Guardian and/or Student are/is aware of the risks to injury, death, and property damage that may result. Among other causes, the active or passive negligence of  Legend Kung Fu Academy  and it’s owners, instructors, employees, assistants, and agents (collectively, “ Legend Kung Fu Academy ”), including without  limitation the  risk of negligent instruction or supervision.  Student  understands that during the course of instruction, employees of  Legend Kung Fu Academy and/or other authorized persons or other students will be engaged in the course of conduct requiring physical contact and he/she gives full consent as is required by the training.

Equipment and the use of equipment:  The Student understands that strict observation of the rules and regulations relative to the training include the use of protective equipment required by the  Legend Kung Fu Academy . The use of protective equipment will minimize the possibility of accidents or injuries. The use of equipment will not eliminate accidents or injuries. The Student assumes all risks of injuries sustained during instruction, class, training, or exercise.

Tuition Policy:   All tuition payments entitle you to the respective number of weeks and classes allotted to each program. Tuition must be renewed upon reaching the allotted number of classes or upon expiration of the enrollment period, whichever occurs first. Belt tests, private lessons, uniforms, and weapons are not covered in this payment.

Make-up Policy:   The student and/or guardian agrees to complete the allotted number of classes within the specified time frame stated above. Students are expected to attend all scheduled classes punctually. In the event of an absence, the instructor must be notified in advance. It is the responsibility of the student and/or guardian to arrange any make-up classes, which are offered at the discretion of the instructor and subject to availability.  Make-up Class Scheduling: To be eligible for a make-up class, students or guardians must notify the instructor at least 24 hours in advance. In cases of emergency or illness, appropriate documentation may be required, and the instructor may grant exceptions at their discretion. Make-up classes must be scheduled before the expiration date noted above and cannot be carried over into a future session or term. Any classes not completed within the designated term will be forfeited.

Release of Photographs:  Guardian and/or student is informed, understands, and acknowledges that the Student may be videotaped, filed, and/or otherwise photographed during the martial arts instruction, training, and /or competition which is the subject of this contract, and hereby, forever grants to  Legend Kung Fu Academy , it’s assigns, licensees, and legal representatives, and their assigns and licensees, including, without limitation, those for whose  Legend Kung Fu Academy  is acting, and all those acting with  Legend Kung Fu Academy  authority and permission, the absolute and irrevocable right and permission to copyright, use and reuse, all photographs, whether made through any media and/or process for art, advertising, trade, promotion, and/or any other purpose whatsoever. Guardian and/or Student also consent to and grants  Legend Kung Fu Academy  the right to the use of any printed material in conjunction with the foregoing. Videotaping is ok during the testing ceremony. However, it is not allowed during the regular class time unless authorized by  Legend Kung Fu Academy.

Release of Liability:  Guardian and/or Student releases Legend Kung Fu Academy  from all liability to Guardian and/or Student and their respective principals, employees, agents, representatives, guardians, successors, assigned heirs, children, and the next to kin for all liability, claims, damages, or demands for personal injuries, death, or property damage occurs on or off the premises. This release includes, without limitations, any personal injuries, death, or property damage caused by the active or passive negligence to the Legend Kung Fu Academy . Buyer and/or Student bears sole and complete responsibility for any loss.
